We accept all hardcopy insurance forms for purposes of medical reimbursement. The forms can be completed by our doctors at the end of the consultation. Please remember to bring a claim form with you on your first visit.
CHI can also offer you a 'cashless' direct billing service. We cooperate on direct billing with the following insurance companies:
For these companies, CHI will deal directly with your insurance company to obtain payment. For all other companies, CHI will try its best to get them on board as soon as possible.

Please note that on your next visit to our clinic, we will complete your details. This first visit cannot be processed with direct billing yet, we need at least 48 hours to get your status confirmed and to set you up for direct billing.
